NEW PATIENT REGISTRATION

New patients who live within the practice boundary are always very welcome. New patients need to complete a registration form. Please phone the surgery on  536 9898 and we will e-mail or post you a registration form. Complete the registration form and e-mail or post it back to us. (make sure you put your telephone number on the form).  The e-mail address to use is repeat.polwarth@nhslothian.scot.nhs.uk. Under normal circumstances we would ask patients to come into the practice to register, however, at this time we would ask that you don't come into the practice to register to avoid having too many people in our waiting room at the same time. Please phone and then use the e-mail address to register at this time. If for any reason you cannot register in this way then you may come into the practice between 10am-12noon,  Mon - Thurs to register. 

Existing patients who move within the practice boundary are welcome to continue as patients but please let us have your new contact details.

As emails travel across the public internet, they can be intercepted. As such, there is no guarantee the e-mails will be delivered to/received by the practice. Practice Boundary

Our practice boundary runs along Home Street, Leven Street, Bruntsfield Place to junction with Colinton Road, west along Colinton Road to junction with Craiglockhart Avenue, along Craiglockhart Avenue, east along Slateford Road to Chesser Avenue junction, along Chesser Avenue, east along Gorgie Road, including Chesser Cottages, Glenlea Cottages to Wheatfield Road junction, along Wheatfield Road, round the back of the distillery and on to Sauchiebank, Haymarket Yards, Haymarket and Morrison Street, over Lothian Road to Bread Street, down through Tollcross and back to Home Street.
